Chairman of the Public Health and Social Services Committee, Craig V. Lopez, and Deputy Chair Peter J. Criswell offer the following:

WHEREAS, this resolution has been submitted at the request of the County Executive on behalf of the Department of Health; and

WHEREAS, the Commissioner of Health requests that the Chair of the Ulster County Legislature be authorized to execute a one-year contract (Contract No. DOH01-T36132GG-3450000) and any future amendments thereto, for the period June 5, 2020 through June 30, 2021, in the amount of $39,153.00 with the New York State Department of Health for LHD Support for Flu and COVID19 Response funds to expand and enhance local health department (LHD) influenza vaccine outreach, promotion and mass vaccination activities for the 2020-21 flu season, as per notification dated November 20, 2020; and

WHEREAS, the Ulster County Department of Health administers the Immunization Action Plan Program to increase childhood immunization and adult immunization rates through education and public awareness campaigns and the LHD Support for Flu and COVID19 Response funds are an extension of this program to provide funding for enhanced flu vaccine promotion in light of the COVID19 pandemic; now, therefore be it

RESOLVED, that the Chair of the Ulster County Legislature is hereby authorized to execute a one-year contract (Contract No. DOH01-T36132GG-3450000) and any future amendments thereto, for the period June 5, 2020 through June 30, 2021, in the amount of $39,153.00 with the New York State Department of Health for LHD Support for Flu and COVID19 Response funds to expand and enhance LHD influenza vaccine outreach, promotion and mass vaccination activities for the 2020-21 flu season, in the form as filed with the Clerk of the Ulster County Legislature or modified with the approval of the County Attorney; and, be it further

RESOLVED, if funding for this program is eliminated, the Commissioner of Finance shall, immediately upon discovery, send written notification to the Clerk of the Legislature, and the relevant Department Head shall attend the next regular meeting

of the Ways & Means Committee to assist the Committee in review and evaluation for determination of the program's continuance,

and move its adoption.
