Chairman of the Public Health and Social Services Committee, Craig V. Lopez, and

Deputy Chairman Chris Allen offer the following:

WHEREAS, this resolution has been submitted by the County Executive on

behalf of the Department of Health; and

WHEREAS, the Commissioner of Health requests that the Chairman of the

Ulster County Legislature be authorized to execute a two-year contract (Contract No.

DOH01-C32484GG-3450000) and any future amendments thereto, for the period

April 1, 2017 through March 31, 2019, in the amount of $80,396.00, with the New

York State Department of Health to continue the Rabies Program; and

WHEREAS, the Rabies Program funds will be used to support the provision

of human rabies treatment, specimen collection and shipment, pet vaccination clinics,

and education and prevention activities; now, therefore, be it

RESOLVED, that the Chairman of the Ulster County Legislature is hereby

authorized to execute a two-year contract (Contract No. DOH01-C32484GG3450000) and any amendments thereto, for the period April 1, 2017 through March

31, 2019, in the amount of $80,396.00, with the New York State Department of

Health to continue the Rabies Program, in the form as filed with the Clerk of the

Ulster County Legislature or as modified with the approval of the County Attorney,

and move its adoption.
