Chairman of the Public Health and Social Services Committee, Craig V. Lopez, and

Deputy Chair Kathy Nolan offer the following:

WHEREAS, this resolution has been submitted by the County Executive on

behalf of the Department of Health; and

 WHEREAS, the Commissioner of Health requests that the Chairman of the

Ulster County Legislature be authorized to execute a contract (Contract No. DOH01-

C32551GG-3450000) and any amendments to the contract with the New York State

Department of Health (NYSDOH) Bureau of Immunization to provide funds to

continue the Immunization Action Plan, for the period of April 1, 2018 through

March 31, 2023, in the amount of $448,065.00; and

 WHEREAS, the contract provides for annual revenue in the amount of

$89,613.00 (CFDA#93.268); and

 WHEREAS, the Ulster County Department of Health will administer these

funds to increase childhood immunization and adult immunization rates through

education and public awareness campaigns; now, therefore, be it

RESOLVED, that the Chairman of the Ulster County Legislature be

authorized to execute a contract (DOH01-C32551GG-3450000) and any amendments

to the contract with the NYSDOH Bureau of Immunization to provide funds to

continue the Immunization Action Plan, for the period of April 1, 2018 through

March 31, 2023, in the form as filed with the Clerk of the Ulster County Legislature

or modified with the approval of the County Attorney; and be it further

RESOLVED, if funding for this program is reduced or eliminated, the

program will be reduced accordingly or eliminated as well,

and moves its adoption.
