Chairman of the Health, Human Services and Housing Committee, Abe Uchitelle, and Deputy Chair Craig V. Lopez offer the following:

 

WHEREAS, this resolution has been submitted at the request of the County Executive on behalf of the Department of Health; and

 

            WHEREAS, the Ulster County Department of Health wishes to obtain and utilize mortality surveillance data to assist in understanding the primary risk factors for the key causes of death of Ulster County residents; and

 

            WHEREAS, the New York State Department of Health, Bureau of Vital Records maintains mortality surveillance data that requires a Data Use Agreement between the state and county to allow access; and

 

WHEREAS, agreement will be in effect for no more than five years and bears no cost: now, therefore, be it

 

RESOLVED, that the Chair of the Ulster County Legislature is authorized to execute a five-year agreement and any amendments thereto, for the period of July 1, 2025 through June 30, 2030 with the New York State Department of Health to grant access to mortality surveillance data,

 

and moves its adoption.
