Chairman of the Health, Human Services and Housing Committee, Abe Uchitelle, and Deputy Chair Craig V. Lopez offer the following:

 

WHEREAS, this resolution has been submitted at the request of the County Executive on behalf of the Department of Health; and

 

WHEREAS, the Commissioner of Health requests that the Chair of the Ulster County Legislature be authorized to execute a contract (Contract No. DOH01-C38477GG-3450000) and any amendments to the contract with the New York State Department of Health (NYSDOH) Bureau of Immunization to provide funds to continue the Immunization Action Plan, for the period of April 1, 2023 through March 31, 2028, in the amount of $457,380.00; and

 

WHEREAS, the contract provides for annual revenue in the amount of $91,476.00 (CFDA#93.268); and

 

WHEREAS, the Ulster County Department of Health will administer these funds to increase childhood immunization and adult immunization rates through education and public awareness campaigns: now, therefore be it

 

RESOLVED, that the Chair of the Ulster County Legislature be authorized to execute a contract (DOH01-C38477GG-3450000) and any amendments to the contract with the NYSDOH Bureau of Immunization to provide funds to continue the Immunization Action Plan, for the period of April 1, 2023 through March 31, 2028, in the form as filed with the Clerk of the Ulster County Legislature or modified with the approval of the County Attorney; and, be it further

 

RESOLVED, if funding for this program is eliminated, the Commissioner of Finance shall, immediately upon discovery, send written notification to the Clerk of the Legislature, and the relevant Department Head shall attend the next regular meeting of the Ways and Means Committee to assist the Committee in review and evaluation for determination of the program’s continuance,       

 

and moves its adoption.
