Chairman of the Public Health and Social Services Committee, Craig V. Lopez, and

Deputy Chairman Chris Allen offer the following:

 

WHEREAS, this resolution was submitted by the County Executive on behalf

of the Department of Social Services; and

WHEREAS, New York State is the recipient of United States Department

of Education funding for Special Education Grants for infants and families

(CFDA 84.181) which is administered as a pass through to local agencies, and

WHEREAS, the Commissioner of the Ulster County Department of Social

Services requests that the Chairman of the Ulster County Legislature be authorized

to enter into an agreement, and any amendments thereto, with the New York State

Department of Health for the Early Intervention Program (NYS Contract No.

C31668GG) in the a n n u a l amount of $69,102 for the five year term beginning

October 1, 2016 and ending September 30, 2021; now, therefore, be it

RESOLVED, that the Chairman of the Ulster County Legislature is hereby

authorized to enter into an agreement, and any amendments thereto, with the New

York State Department of Health for the Early Intervention program (NYS Contract

No. C31668GG) in the annual amount of $69,102.00, for the term of October 1, 2016

through September 30, 2021, in the form as filed with the Clerk of the Ulster County

Legislature or as modified with the approval of the County Attorney; and be it further

RESOLVED, that if funding for this program is eliminated, the program

will be eliminated as well,

and move its adoption.
