Chairman of the Public Health and Social Services Committee, Craig V. Lopez, and

Deputy Chairman Jeanette Provenzano offer the following:

WHEREAS, this resolution has been submitted by the County Executive

on behalf of the Department of Health; and

WHEREAS, the Commissioner of Health requests that the Chairman of

the Ulster County Legislature be authorized to execute a three-year contract

(Contract No. C029747) for the period October 1, 2014 through September 30,

2017, in the amount of $76,254.00 with the New York State Department of

Health to continue the Children With Special Health Care Needs (CSHCN)

Program; and

WHEREAS, the Ulster County Department of Health administers the

CSHCN Program, providing families with information and referrals to insurance

and health services and community resources to address their identified special

health care needs; now, therefore, be it

RESOLVED, that the Chairman of the Ulster County Legislature is

hereby authorized to execute a three-year contract (Contract No. C029747) and

any amendments thereto, for the period from October 1, 2014 through

September 30, 2017, in the amount of $76,254.00, with the New York State

Department of Health to continue the CSHCN Program, in the form as filed with

the Clerk of the Ulster County Legislature or modified with the approval of the

County Attorney; and, be it further

RESOLVED, that if funding for this program is eliminated, the program will

be eliminated as well,

and move its adoption.
