Chairman of the Public Health and Social Services Committee, Craig V. Lopez, and Deputy Chair Peter J. Criswell offer the following:

 

WHEREAS, this resolution has been submitted by the County Executive’s Office on behalf of the Department of Health; and

 

WHEREAS, the Commissioner of Health requests that the Chair of the Ulster County Legislature be authorized to execute a five-year contract (Contract No. DOH01-C35750GG-3450000) and any future amendments thereto, for the period October 1, 2020 through September 30, 2025, in the amount of $144,505.00 with the New York State Department of Health to continue the Children and Youth with Special Health Care Needs (CYSHCN) Program; and

 

WHEREAS, the Ulster County Department of Health administers the CYSHCN Program, providing families with information and referrals to insurance and health services and community resources to address their identified special health care needs; now, therefore, be it

 

RESOLVED, that the Chair of the Ulster County Legislature is hereby authorized to execute a five-year contract (Contract No. DOH01-C35750GG-3450000) and any future amendments thereto, for the period from October 1, 2020 through September 30, 2025, in the amount of $144,505.00 with the New York State Department of Health to continue the CYSHCN Program, in the form as filed with the Clerk of the Ulster County Legislature or modified with the approval of the County Attorney; and, be it further

 

RESOLVED, if funding for this program is eliminated, the Commissioner of Finance shall, immediately upon discovery, send written notification to the Clerk of the Legislature, and the relevant Department Head shall attend the next regular meeting of the Ways & Means Committee to assist the Committee in review and evaluation for determination of the program's continuance,

 

 

 

and move its adoption.
