Chairman of the Public Health and Social Services Committee, Craig V. Lopez, and Deputy Chair Kathy Nolan offer the following:

 

WHEREAS, this resolution has been submitted by the County Executive on behalf of the Department of Health; and

 

WHEREAS, the Commissioner of Health requests authorization to sign an annual renewal provider agreement and profile with the New York State Department of Health to continue participation in the Vaccines For Children Program; and

 

WHEREAS, participation in the Vaccines For Children Program qualifies the Department of Health to receive publicly funded vaccines at no cost for administration to children meeting Vaccines for Children Program eligibility requirements as determined by federal and/or state guidelines; now, therefore, be it

 

RESOLVED, that the Commissioner of Health is authorized to sign an annual renewal provider agreement and profile, with the New York State Department of Health to continue participation in the Vaccines For Children Program, in the form as filed with the Clerk of the Ulster County Legislature or as modified with the approval of the County Attorney, 

 

and move its adoption.
