Chairman of the Legislative Programs, Education and Community Services

Committee, David B. Donaldson, and Deputy Chairman Mary Wawro offer the

following:

WHEREAS, this resolution has been submitted by the County Executive on

behalf of the Ulster County Office for the Aging; and

WHEREAS, the New York State Office for the Aging has notified Ulster

County that funding for the 2014-2015 Medicare Improvements for Patients and

Providers Act (MIPPA) program is available in the amount of $11,192.00; and

WHEREAS, the goal of the program is to provide outreach and assistance to

Medicare Part D beneficiaries in rural areas, and for activities related to the

promotion and distribution of information on Medicare preventive and wellness

services; and

WHEREAS, the term for the MIPPA program year is September 30, 2014

through September 29, 2015; and

 

WHEREA

 

RESOLVED, that the Chairman of the Ulster County Legislature is hereby

authorized to execute the application with required assurances, and any amendments

thereto, with the New York State Office for the Aging for the Medicare

Improvements for Patients and Providers Act program, for the term beginning

September 30, 2014 through September 29, 2015, in the form as filed with the Clerk

of the Ulster County Legislature or as modified with the approval of the County

Attorney;

and move its adoption.
